What is oral histology and what is its scope?
Oral histology is the microscopic study of the structure of oral tissues (teeth, periodontium, oral mucosa, salivary glands, TMJ). Its scope covers their development, normal microscopic architecture, and the structure-function relationships clinically relevant to dentistry.
Why is decalcification required before sectioning mineralized dental tissues, and which agents are used?
Mineral content makes hard tissues too brittle to cut with a microtome. Decalcification removes calcium salts using acids (nitric, formic) or chelating agents (EDTA); EDTA is slower but best preserves tissue morphology.
How are ground sections prepared and why are they used in dental histology?
Undecalcified teeth are sliced and abraded/polished to a thin (~30 µm) translucent section. They preserve enamel and mineralized detail (e.g., enamel rods, incremental lines) that decalcification destroys.
What is the routine stain used in histology and what does each component stain?
Hematoxylin and Eosin (H&E). Hematoxylin stains acidic/basophilic structures (nuclei, RNA) blue-purple; eosin stains basic/acidophilic structures (cytoplasm, collagen) pink.
From which embryonic germ layers and structures do the dental tissues derive?
Enamel derives from ectoderm (oral epithelium/enamel organ). Dentin, pulp, cementum, and periodontal ligament derive from ectomesenchyme (neural crest). The supporting alveolar bone also arises from ectomesenchyme.
Name the stages of tooth development in sequence.
Initiation (dental lamina) → bud stage → cap stage → bell stage → apposition/crown stage → root formation and eruption.
What is reciprocal induction in tooth development?
A series of signaling interactions between the dental epithelium (enamel organ) and the underlying ectomesenchyme (dental papilla) in which each tissue alternately induces differentiation in the other, driving tooth morphogenesis.
What is the function of the enamel knot in the cap/bell stage?
The enamel knot is a transient signaling center (secreting BMP, FGF, SHH, WNT) that controls cusp position and tooth crown shape, then disappears by apoptosis.
Which signaling pathways/genes are key genetic regulators of odontogenesis?
BMP, FGF, SHH, and WNT pathways, with transcription factors such as MSX1, PAX9, PITX2, and RUNX2 coordinating tooth initiation and morphogenesis.
Which gene mutations are associated with hypodontia/oligodontia (missing teeth)?
Mutations in MSX1, PAX9, AXIN2, and WNT10A are classically associated with familial hypodontia and oligodontia.
What is the inorganic, organic, and water composition of mature enamel by weight?
About 96% inorganic (hydroxyapatite), and roughly 4% organic matrix and water combined. It is the hardest and most highly mineralized tissue in the body.
What are the two main enamel matrix proteins and their roles?
Amelogenins (~90%, regulate crystal growth/thickness and spacing) and non-amelogenins such as enamelins and ameloblastins (control crystal nucleation and elongation). Both are largely removed during maturation.
What is the basic structural unit of enamel and its shape?
The enamel rod (prism), running from the dentinoenamel junction toward the surface. In cross-section it is keyhole-shaped, with the head pointing occlusally and the tail cervically.
What are the Striae of Retzius and the Lines of Retzius?
They are incremental growth lines of enamel reflecting successive apposition. The neonatal line is an accentuated stria marking the stress of birth, seen in enamel formed before and after birth.
Define enamel spindles, enamel tufts, and enamel lamellae.
Spindles: odontoblast processes crossing the DEJ. Tufts: hypomineralized inner-third projections resembling grass tufts. Lamellae: thin hypomineralized sheets extending from surface toward the DEJ.
What is the inorganic vs organic/water composition of dentin?
About 70% inorganic (hydroxyapatite), 20% organic (mainly type I collagen), and 10% water by weight. It is harder than bone/cementum but softer than enamel.
What are dentinal tubules and what do they contain?
S-shaped (primary curvature) channels running from the pulp to the DEJ, each containing an odontoblast process and dentinal fluid; they account for dentin's permeability and sensitivity.
Differentiate primary, secondary, and tertiary (reparative) dentin.
Primary dentin forms before root completion. Secondary dentin forms slowly throughout life after eruption, narrowing the pulp. Tertiary (reparative/reactionary) dentin forms locally in response to injury such as caries or attrition.
