  1. What is the medico-legal definition of death as per the Transplantation of Human Organs Act, including brain-stem death?

    Death is the permanent disappearance of all evidence of life, i.e., irreversible cessation of brain-stem function (brain-stem death) or irreversible stoppage of circulation and respiration. Brain-stem death must be certified by a board of 4 doctors with two examinations 6 hours apart.

  2. List the four classical signs/changes used to estimate the time since death (post-mortem interval).

    Algor mortis (cooling of body), rigor mortis (muscle stiffening), livor mortis/post-mortem lividity (hypostasis), and decomposition/putrefaction. Stomach contents and entomology also help.

  3. Describe rigor mortis: when it appears, the order it progresses, and how long it lasts in a temperate climate.

    Rigor mortis appears 1-2 hours after death, is complete in about 12 hours, persists for about 12 hours, and passes off in another 12 hours. It follows Nysten's rule, starting in the eyelids/jaw and descending downward, then disappearing in the same order.

  4. What is cadaveric spasm and how does it differ medico-legally from rigor mortis?

    Cadaveric spasm (instantaneous rigor) is instantaneous stiffening of a group of voluntary muscles at the moment of death, without preceding primary flaccidity. Unlike rigor mortis it is not preceded by relaxation and cannot be artificially produced, so it indicates the act (e.g., grasping a weapon) was performed during life.

  5. State the average rate of fall of body temperature after death and the formula used to estimate time since death from temperature.

    Body cools at roughly 1.5°F (about 0.8°C) per hour in temperate climates. Approximate time since death = (Normal body temp 98.4°F - Rectal temp) / Rate of fall per hour. Cooling follows a sigmoid curve, not linear.

  6. Differentiate post-mortem lividity (hypostasis) from a bruise (contusion).

    Lividity: appears in dependent areas, intravascular blood, no swelling/abrasion, color uniform, disappears on incision (blood washes away). Bruise: anywhere, extravascular blood, swelling present, can occur over bony points, does not wash away on incision and tissues are infiltrated.

  7. What is the medico-legal significance of stomach contents in estimating time since death?

    Stomach empties in about 4-6 hours after a meal. Presence of undigested food suggests death within ~2 hours of eating; partly digested food ~2-4 hours; empty stomach with empty duodenum suggests death several hours after the last meal.

  8. Name the early changes of putrefaction and the first external sign.

    The first external sign of putrefaction is greenish discoloration of the skin over the right iliac fossa (caecum), appearing 12-24 hours after death. It is due to sulphmethaemoglobin formation by bacterial action.

  9. What is the difference between adipocere and mummification?

    Adipocere: hydrolysis and hydrogenation of body fat into a greasy, waxy substance, requires moisture and warmth, takes weeks to months. Mummification: dehydration/desiccation of the body, requires a dry, hot environment with good air circulation, body becomes shrunken and leathery.

  10. List the components of a medico-legal autopsy (postmortem examination) protocol and the standard incisions.

    It includes external examination, then internal examination of the cranial, thoracic, and abdominal cavities. Standard incisions: I-shaped (straight midline), Y-shaped, or modified Y; for the head a coronal/intermastoid incision is used to reflect the scalp.

  11. What is the purpose of the negative autopsy and the obscure autopsy?

    Negative autopsy: no cause of death found even after complete autopsy plus histology, toxicology, and microbiology. Obscure autopsy: anatomical findings are absent or non-specific/insufficient to explain death; cause established only by ancillary investigations.

  12. State the test for live birth: the hydrostatic (Raygat's) lung float test and its limitations.

    Lungs that have respired float in water (contain air, lower specific gravity), indicating live birth. Limitations: putrefaction gases cause false float; atelectasis or partial respiration may cause unrespired-appearing lungs to sink; artificial respiration can introduce air.

  13. Define asphyxia and classify the asphyxial deaths.

    Asphyxia is the condition caused by interference with respiration leading to hypoxia and hypercapnia. Types: mechanical asphyxia (hanging, strangulation, throttling, smothering, gagging, choking, traumatic/postural asphyxia, drowning), and others. Mechanical violent asphyxial deaths are the classic forensic group.

    Hanging: mark is high up the neck above the thyroid, oblique, non-continuous (gap at knot), runs upward toward the knot, base pale and hard. Strangulation: mark low (at/below thyroid), horizontal, continuous around the neck, base soft.

  15. What are the classic internal findings and the medico-legal significance of a fractured hyoid bone in neck compression?

    Hyoid bone fracture (greater cornua) and thyroid cartilage fractures indicate significant force on the neck, more common in manual strangulation (throttling) and in older individuals (ossified hyoid). It supports antemortem violence to the neck.

  16. List the signs of throttling (manual strangulation) on the neck.

    Multiple bruises and crescentic fingernail abrasions over the neck, especially around the larynx; bruising of underlying neck muscles; fracture of hyoid bone and thyroid cartilage; petechial haemorrhages in eyes and face; congestion and cyanosis.

  17. Differentiate antemortem drowning from postmortem submersion.

    Antemortem: fine leathery froth at mouth/nostrils, water and weeds in deeper airways and stomach, washerwoman's hands, cutis anserina, ballooned waterlogged lungs (emphysema aquosum), diatoms in distant organs. Postmortem: no froth, no diatoms in marrow, no fluid in deep airways.

  18. What is the Gettler test and the diatom test in drowning?

    Gettler test: compares chloride content of blood from the left and right sides of the heart (now unreliable). Diatom test: demonstration of diatoms (silica-shelled algae) in bone marrow and distant organs proves antemortem drowning, as they enter circulation only with a beating heart.
